Rationale, aims and objectives Given the increasing prevalence of cataract and demand for cataract extraction surgery, patients must often wait to undergo this procedure. We validated a previously developed priority scoring system in terms of clinical variables, pre-intervention health status, appropriateness of surgery and gain in visual acuity (VA) and health-related quality of life (HRQoL).Methods Explicit prioritization criteria for cataract extraction created by a variation of the Research and Development (RAND) and University of California Los Angeles appropriateness methodology were retrospectively (...) applied to a prospective cohort of 5257 patients on waiting lists to undergo cataract by phacoemulsification at 17 hospitals in Spain. Demographic data, clinical data and data related to surgical technique were collected by trained ophthalmologists. Patients were evaluated by their ophthalmologist before the intervention and 6 weeks afterward. They also completed, by mail, the Visual Function Index (VF-14) before the intervention and 3 months afterward.Results High-priority patients experienced greater improvement in VA and HRQoL than those classified as intermediate or low-priority (P < 0.0001), even after adjusting by VA and the VF-14 score at baseline. The time to intervention was the same for high-priority patients as it was for intermediate and low-priority patients.Conclusions The priority score we developed identified patients most likely to experience the greatest improvements from cataract extraction. Use of this tool could provide a fairer and more rational way to prioritize patients for cataract extraction. (shrink)

Music can reduce stress and anxiety, enhance positive mood, and facilitate social bonding. However, little is known about the role of music and related personal or cultural variables in maintaining wellbeing during times of stress and social isolation as imposed by the COVID-19 crisis. In an online questionnaire, administered in 11 countries, participants rated the relevance of wellbeing goals during the pandemic, and the effectiveness of different activities in obtaining these goals. Music was found to be the most effective activity (...) for three out of five wellbeing goals: enjoyment, venting negative emotions, and self-connection. For diversion, music was equally good as entertainment, while it was second best to create a sense of togetherness, after socialization. This result was evident across different countries and gender, with minor effects of age on specific goals, and a clear effect of the importance of music in people's lives. Cultural effects were generally small and surfaced mainly in the use of music to obtain a sense of togetherness. Interestingly, culture moderated the use of negatively valenced and nostalgic music for those higher in distress. (shrink)

Aberrations introduced by the atmospheric turbulence in large telescopes are compensated using adaptive optics systems, where the use of deformable mirrors and multiple sensors relies on complex control systems. Recently, the development of larger scales of telescopes as the E-ELT or TMT has created a computational challenge due to the increasing complexity of the new adaptive optics systems. The Complex Atmospheric Reconstructor based on Machine Learning is an algorithm based on artificial neural networks, designed to compensate the atmospheric turbulence. During (...) recent years, the use of GPUs has been proved to be a great solution to speed up the learning process of neural networks, and different frameworks have been created to ease their development. The implementation of CARMEN in different Multi-GPU frameworks is presented in this paper, along with its development in a language originally developed for GPU, like CUDA. This implementation offers the best response for all the presented cases, although its advantage of using more than one GPU occurs only in large networks. (shrink)

Is the societal-level of analysis sufficient today to understand the values of those in the global workforce? Or are individual-level analyses more appropriate for assessing the influence of values on ethical behaviors across country workforces? Using multi-level analyses for a 48-society sample, we test the utility of both the societal-level and individual-level dimensions of collectivism and individualism values for predicting ethical behaviors of business professionals. Our values-based behavioral analysis indicates that values at the individual-level make a more significant contribution to (...) explaining variance in ethical behaviors than do values at the societal-level. Implicitly, our findings question the soundness of using societal-level values measures. Implications for international business research are discussed. (shrink)

Background: Serious games have shown positive results in increasing motivation, adherence to treatment and strengthening the therapeutic alliance in multiple psychiatric disorders. In particular, patients with impulse control disorders and other disorders in which the patient suffers from inhibitory control deficits have been shown to benefit from serious games.Aim: The aim of this study was to describe the characteristics and to evaluate the usability of a new serious videogame, e-Estesia. This serious videogame was designed to improve emotion regulation in patients (...) with gambling disorder. Preliminary results from a pilot sample are also reported.Method: A pilot sample of 26 patients undergoing treatment for GD was recruited. Participants used e-Estesia on a tablet, which was connected to a thoracic band that sent heart rate and heart rate variability data to the videogame platform in order to provide biofeedback. The System Usability Scale was completed by patients to determine the usability of e-Estesia.Results and Discussion: e-Estesia performed comparatively well for all the explored groups. Around 84.6% of the patients endorsed that it was easy to use. Female patients with GD presented higher HRV during the use of the serious videogame compared to men. (shrink)

Diabetes has been associated with affective disorders which complicate the management of the disease. Emotional intelligence, or the ability to perceive, facilitate, understand, and regulate emotions, has shown to be a protective factor of emotional disorders in general population. The main objective of this study was to systematically review the role of the EI construct in Type 1 and Type 2 diabetics and to observe how EI is related to biological and psychological variables. Comprehensive searches were conducted in PubMed, Scopus, (...) PsycInfo, and Cochrane without time limitations, for studies examining the link between diabetes and EI. A total of 12 eligible studies were selected according to the inclusion criteria. We divided the results into four sections: EI and hemoglobin glycosylated, EI training effects, differences in EI between persons with diabetes and without diabetes, and EI and psychological adjustment and well-being. The results showed negative correlations between EI and HbA1c, positive effects of EI training on quality of life, anxiety, and glycemic control, no differences in EI between people with diabetes and healthy individuals, and, finally, negative correlations between EI and different psychological variables such as diabetes-related anxiety and distress, and positive correlations with quality of life, well-being, and marital satisfaction. This systematic review offers a starting point for a theoretical and practical understanding of the role played by EI in the management of diabetes and reveals that EI is a promising protective factor for biological and psychological variables in this population. Interest in ethical aspects associated to product acquisition and consumption is a growing trend among consumers. In this context, the concept of “product with ethical attributes” has arisen to refer to products with explicit social and environmental characteristics. However, one of the factors that most hinders the purchasing of these products is certainly price. Given the difficulty of reducing price, the question that arises is the extent to which other product attributes can attenuate the negative impact of price on perceived (...) value. We assume that the special benefits associated to this type of products are, at a different level, attenuators of the relationship between price and perceived value. Focusing on Fair Trade organic coffee, hypotheses are tested regarding survey data from 407 customers. They were interviewed in an actual purchasing scenario. The models are tested using conventional Structural Equation Models and the Latent Moderated Structural Equation method. The results obtained shed some light on a highly generalised belief that the marketing of these products can only be improved by reducing their price. However, although a price reduction could be desirable, albeit complicated in this product category, its effects could be reduced by acting on other variables such as the ethical aspects, quality and healthiness of this product category. This article shows the general guidelines of the rortyan lecture of Kant, focused in his role as the highest representative of the conversion of philosophy in epistemology, and also some criticism to that lecture.

Among the elements which have gradually been complicating the countless representations of the Virgin Mary throughout history, this paper seeks to highlight and interpret conceptually one of special doctrinal significance in some Marian images during the fourteenth and fifteenth centuries: the temple, in whose interior some artists place some actual or symbolic episodes of Mary, from her birth or her Annunciation to the Sacra Conversazione, to give a few examples. Even though at first sight it looks as a mere scenographic (...) or ornamental ingredient, without iconographic value, this precise shape of temple reveals in fact some profound Mariological and Christological meanings. In that order of ideas, this essay is based on a double methodological strategy: first and foremost, to analyze some Marian images which include in the aforementioned period the shape of a temple, in order to see what kind of role such a religious building plays in them; secondly, to discover the relevant keys for interpreting such images, by analyzing some specific texts of the Church Fathers and medieval theologians who identify with Mary the figure of the «temple of God». For this reason, after a brief introduction to the problem, the present study is structured into three main parts inextricably intertwined: in the first we comparatively analyze twelve Marian paintings of the fourteenth and fifteenth centuries in which the Virgin appears inside a temple; in the second we explain the many exegesis that, in a consistent and strong patristic and theological tradition, identify the analogy of the templum Dei directly with the Virgin Mary, or indirectly with the Christ’s human body conceived in her womb; on the basis of this double analysis of images and exegetical writings, the third part of the paper states as a synthetic conclusion that the artistic representation of a temple or a church building in some Marian images of this period offers a compelling plastic metaphor of Mary and, ultimately, of her virginal divine motherhood. La tesis mariológica de la conceptio per aurem, según la cual la Virgen María habría concebido a Jesucristo por el oído en el momento de escuchar del ángel el mensaje celestial anunciándole que, sin perder su virginidad, sería madre del Hijo de Dios encarnado, ha merecido hasta ahora muy pocos estudios académicos rigurosamente fundados en fuentes primarias. De hecho, en la literatura especializada son muy escasas las referencias a tal teoría y, cuando algún estudioso la evoca, casi siempre se (...) contenta con aludir a ella, sin aportar pruebas documentales. Sin embargo, tal como lo revelan las nueve pinturas italianas aquí analizadas, esa teoría fue ilustrada mediante sutiles metáforas visuales en muchas obras pictóricas medievales, las cuales se inspiraron en una sólida tradición literaria. Además una pléyade de Padres de la Iglesia y teólogos medievales testimonia, mediante afirmaciones explícitas, que semejante teoría gozó de notable aceptación entre los maestros del pensamiento cristiano. Basándose en numerosos textos patrísticos y teológicos, este artículo intenta dos objetivos esenciales: exponer, ante todo, las distintas formulaciones teóricas propuestas por esos pensadores; y además, tratar de poner en luz los significados dogmáticos que subyacen bajo esa sorprendente tesis. Major depression etiopathogenesis is related to a wide variety of genetics, demographic and psychosocial factors, as well as to environmental factors. The objective of this study is to analyze sociodemographic and environmental variables that are related to the prevalence of depression through correlation analysis and to develop a regression model that explains the behavior of this disease from an ecological perspective. This is an ecological, retrospective, cross-sectional study. The target population was 1,148,430 individuals over the age of 16 who were (...) registered in Aragon (Spain) during 2010, with electronic medical records in the community’s primary health care centers. The spatial unit was the Basic Health Area (BHA). The dependent variable was the diagnosis of Depression and the ecological independent variables were: Demographic variables (gender and age), population distribution, typology of the entity, population structure by sex and age, by nationality, by education, by work, by salary, by marital status, structure of the household by number of members, and state of the buildings. The results show moderate and positive correlations appear with higher rates of depression in areas having a higher femininity index, higher population density, areas with a higher unemployment rate and higher average salary. The results of the linear regression show that aging +75 and rural entities act as protective factors for depression, while urban areas and deficient buildings act as risk factors. In conclusion, the ecological methodology may be a useful tool which, together with the statistical epidemiological analysis, can help in the political decision making process. (shrink)

Está surgiendo una nueva cultura tecnologías, aunque dominada por el capitalismo como sistema director. La cuestión dominante de este documento es la siguiente: ¿qué personalidad tiende a conformar esta nueva cultura? Si atendemos a sociólogos, especialmente a los sociólogos americanos, que nos hablan de "realidad virtual", deberíamos decir que se trata de una personalidad cuya percepción de (...) lo real está seriamente dañada. Esto imposible que se conecte con la verdadera realidad, que es el otro hombre, en tanto que persona y fin en sí mismo. (shrink)

Durante los últimos decenios del siglo XX la filosofía de Francisco Suárez fue considerada parte integrante de un movimiento escolástico comúnmente mencionado como escuela tomista. La perspectiva de abordaje, según el mote impuesto, colocaba la posición especulativa del Dr. Eximio más próxima al dominico Tomás de Aquino de acuerdo con los siguientes matices: a. en el ámbito epistémico la figura del Aquinate eclipsaba ampliamente aquella de Suárez, promoviendo una auténtica desconsideración de sus tesis originales; b. el jesuita español fundaba, igual (...) que Tomás de Aquino, la división de las ciencias especulativas en el acto intelectual de abstracción; c. la diferencia entre ambas concepciones metafísicas no implicaba necesariamente una comprensión epistémica distinta. Ahora bien, contra estas interpretaciones aparece en el siglo XXI otra lectura, donde se acentúa la diferencia decisiva de la filosofía del jesuita del siglo XVI, quien, en letras medievales, escribe y piensa en permanente tensión entre la herencia de la filosofía antigua y medieval por una parte, y una neo-escolástica de aires renacentistas por la otra. De acuerdo con ello, si bien su pensamiento discurre por una tradición aristotelizante del saber, la comprensión de las ciencias se asienta, a diferencia de Tomás de Aquino, en clave objetiva. Sobre esta premisa, el siguiente estudio abordará con exclusividad la originalidad epistémica de Suárez según la división de las ciencias especulativas y reales, mostrando el orden, el principio y las relaciones entre las ciencias física, matemática y metafísica. Escribir hoy en día un libro sobre hermenéutica, que tal hermenéutica se refiera a la desarrollada por G. Gadamer en su conocido Verdad y método y que se pretenda añadir algo nuevo a lo mucho escrito sobre el tema parecería, a primera vista, empresa irrealizable. Que ambas pretensiones inspiren la sólida monografía de María G. Navarro —titulada Interpretar y argumentar— constituye empresa audaz y arriesgada, plena de coraje innovador, que provoca admiración, curiosidad e interés. Contra lo que pudiera parecer (...) a primera vista, el libro contiene un alto componente de originalidad y creatividad, debido a la estratagema metodoló-gica de que se sirve la autora. A saber, una hermenéutica in obliquo, estrategia consistente en interpretar a la hermenéutica gadameriana a través del prisma de la lógica de la argumentación. (shrink)

In this paper, I argue that the certainty about the wrongness of killing must not be considered as a universal, but as a local one. Initially, I show that there exist communities in which the wrongness of killing innocents is not a moral certainty and that this kind of case cannot be justified by arguing that such people are psychopaths. Lastly, I argue that universal certainties do not admit of exceptions: thus, the fact that some exceptional cases affect the certainty (...) that killing innocents is wrong, leads me to conclude that it is a local certainty. Mucho se ha hablado de la «existencia» como eje de la filosofía kierkegaardiana, pero muy poco se ha dicho sobre la «idea» que la sostiene y promueve. En efecto, existir es para Kierkegaard la reflexión concreta de la idealidad, la manifestación efectiva de un concepto, que avanza dialécticamente desde su indeterminación abstracta y formalmente posible hasta la conciencia absoluta de su identidad. Las siguientes páginas se proponen, en primer lugar, mostrar el dinamismo circular de la idea como fundamento, medio y (...) fin de la existencia kierkegaardiana. En segundo lugar, ellas intentarán contribuir a la rectificación de una lógica dualista, que ha contrapuesto de manera irreconciliable idea y realidad. (shrink)
